Join us for our annual meeting on October 2nd at Park Central Hotel in San Francisco, CA. The business meeting and CE programming is planned to begin at noon with a reception to follow at 6:00 pm.

Registration Information 

This program is free to SIDP members. You must register online in advance of attending this program at www.sidp.org. However, if you are a member and register on-site there will be a $25 on-site registration fee for the meeting and a $25 fee for the reception. In addition, those who register prior to the meeting but fail to attend and do not cancel at least 48 hours prior to the program you will be charge $50 to cover expenses for meeting and $75 to cover expenses for reception. If you do attend the meeting or cancel in the specified time, you will not be charge the fee. Please register online no later than September 14, 2018. Fee for non-members/non-healthcare professionals is $125 for the meeting and evening reception or $75 for the evening reception only. Checks should be mailed to: SIDP, 121 W State St, Geneva, IL 60134.
