POSITION SUMMARY

The Internal Medicine/Antimicrobial Stewardship Clinical Pharmacist Specialist provides comprehensive pharmaceutical care to all patients through integrated drug distribution, clinical services, and education.

POSITION REQUIREMENTS

Formal Education / Training:

Earned a Pharmacy diploma or degree from an ACPE accredited college or university. Completed an ASHP accredited PGY-2 Internal Medicine or Infectious disease residency or PGY-1 Pharmacy Practice residency with three years relevant clinical experience or five years relevant clinical experience. Must have a current license from the State of Tennessee. Board certification in Infectious Disease within one year of hire. ACLS within six months of hire.

Workplace Experience:

Hospital or related experience required.

Equipment and Skills Training:

Meditech Pharmacy Module computer system, Automated Dispensing Cabinet Workstation, calculators, personal computers and printers, laminar flow hood, PDAs.

Physical Environment:

Work in an air-conditioned room and/or clean room or office. View and accurately type on a computer terminal for at least a two hour period. Ability to handle frequent interruptions and adapt to changes in workload and work schedule.

Physical Effort:

View and accurately type on a computer terminal for at least a two hour period. Competently work in a seated position for a two hour period. Efficiently communicate both orally and in writing. Safely and efficiently stand, stoop, bend, reach and lift in order to maintain the department equipment and supplies. Excellent communication; teaching skills; ability to communicate effectively with people and other health care professionals. The ability to prioritize and manage both individuals and

projects/programs. The ability to communicate effectively with people with varying degrees and types of disabilities. The ability to adjust anticoagulation therapy.

Occupational Hazards:

Potential for exposure to hazardous and toxic substances (including chemotherapeutic, cytotoxic drugs and cleaning solutions), sticks or cuts by needles and other sharp items. Potential for musculoskeletal injuries if proper lifting and carrying techniques are not used.

Customer Service:

Must practice WMC’s Standards of Performance Service Guidelines and Standards. Must be sensitive to the needs of patients, families, visitors, co-workers and staff. Must possess an extremely pleasant, courteous and helpful attitude; excellent communication skills and the ability to work closely and cooperatively with others is required. Ability to set priorities and solve problems. Fosters a team environment by providing orientation and training to new team members. Ability to communicate effectively (verbal and written). Ability to meet the pharmaceutical care needs of neonatal, pediatric, adolescent, adult, geriatric, obstetric, and oncology,

PERFORMANCE STANDARDS

1. Provide operational support on the floors/wards and Central Pharmacy as determined by departmental need; Review medication orders for accuracy and appropriateness; Order entry for assigned areas; Resolves drug distribution related issues; Resolves automated dispensing cabinet related issues; Covers assigned shifts as determined by departmental need, including evenings, weekends and holidays

2. Discuss medication order clarifications with the prescriber, document any changes in patient and pharmacy records, and inform others of medication order changes

3. Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions

4. Prevent, detect, monitor, document, and report adverse drug reactions and medications errors. Participate in the drug therapy management in emergencies situations. Identify and conduct patient safety and process improvement initiative as need arises.

5. Promote the use of the formulary by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary drugs when necessary. Conduct drug formulary reviews and present to appropriate committees.

6. Promote and demonstrate rational, cost-effective drug therapy. Identify and implement drug therapy where positive pharmacoeconomic impact could be achieved. The ultimate goal is to develop and assure cost effective clinical pathways.

7. Leads Antimicrobial Stewardship Program Team and serves as pharmacy chair. Responsible for regulatory reporting of antibiotic use through NHSN; responsible for prospective review of patients on antibiotics and blood culture results; facilitates the activities of the pharmacists with regards to antibiotic stewardship. Works with hospital epidemiologist/infectious disease specialist on appropriate use of hospital antibiogram and antimicrobial agents.

8. Develop and conduct target drug programs and drug usage evaluations as needed and report results to the Pharmacy and Therapeutics Committee. Participate as an active member of Pharmacy and Therapeutics Committee.

9. Document clinical interventions and cost avoidance as part of the departmental documentation program. Monitor the use of expensive medications to assure there is use consistent with approved criteria.

10. Anticipate changes in drug therapy and report the potential impact it will represent to the overall hospital and drug budget.

11. Take a leadership role in the teaching programs of the Department of Pharmacy Services and Colleges of Pharmacy as they are relate to Internal Medicine and Antimicrobial Stewardship, precept students and residents. Contribute and assist in editing the monthly pharmacy newsletter.

12. Provide staff development support to nurses and physicians.
