    This joint position will support the continued advancement of the Anti-infective Stewardship Programs (ASP) at Massachusetts General Hospital (MGH). This ID Clinical Pharmacist will help advance the mission of the program while:

    • Serving as a liaison between the MGH Infectious Diseases and Internal/Hospital Medicine pharmacy teams
    • Providing direct patient care within the multi-disciplinary Infectious Diseases and Med-Surg inpatient teams at MGH.

    Clinical Pharmacists assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Clinical Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management within specialty patient care services and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacists serve as a departmental resources and liaisons to other departments, hospital personnel, or external groups. They proficiently manage and facilitate the appropriate use of medications according to professional and regulatory standards. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice.     Licenses, Certifications, and/or Registrations:   

    • Current licensure as a pharmacist in the state of Massachusetts (or eligible) required
    • Board Certification in area of expertise is required within 18 months of hire (if not already certified)


    • Completion of ASHP-accredited PGY1 Pharmacy Residency required and PGY2 Infectious Diseases Pharmacy Residency or Fellowship preferred

    Pharmacist Position

    Description of job:

    The following responsibilities are divided between 2 ID pharmacists. Restructuring of responsibilities will be considered based on the candidate’s interests.

    Inpatient responsibilities include attending rounds with the consultation team, performing antimicrobial stewardship (prospective audit and feedback) and assessing restricted antimicrobial drug requests. Outpatient responsibilities include monitoring of OPAT patients, assisting ID physicians in clinic, as well as attending ID pharmacy specific clinics (in person and phone) where patients are seen for hepatitis C, latent TB, azole TDM, latent TB drug monitoring and HIV/PrEP drug monitoring.

    Administrative responsibilities include sitting on various committees (PNT, Infection control, antimicrobial subcommittee, VISN12 hepatitis task force, VISN12 stewardship task force), participating in QI/QA projects as well as maintaining and creating antimicrobial related policies. ID pharmacists precept pharmacy students and PGY1 resident projects and while on rotation. The ID pharmacy section also has a PGY2 infectious diseases pharmacy residency program with one resident.

    Assistant Professor of Clinical Pharmacy – Adult Infectious Diseases

    We are recruiting for an Assistant Professor of Clinical Pharmacy – Adult Infectious Diseases (non-tenure track). The practice site for the position is Cooper University Hospital, in Camden, NJ (8 miles from campus) and includes both inpatient ID consult service, as well as outpatient (e.g. HIV) clinic opportunities. This individual will be responsible for precepting pharmacy students and residents on experiential (IPPE and APPE) rotations and will have the opportunity to precept PGY2 residents as well. Additionally, the individual will assist with on-campus teaching, student mentoring, and student precepting at the Cooper Rowan Clinic (an interprofessional student managed clinic).       Position Summary: This pharmacist will assist the Associate Director for Antimicrobial Stewardship and the Medical Director of Antimicrobial Management with the task of improving antimicrobial utilization.  Through collaborative work efforts, the person in this position will also help to implement processes to improve the clinical and microbiological effectiveness of antimicrobials.  This will include use of computer systems including computerized provider order entry, microbiology and online medical records.   This candidate will assist in the tracking and trending of antimicrobial resistance particularly as it relates to utilization.  He/she will participate in the education of prescribers and other healthcare providers including pharmacists and nurses toward improvements in the process of maximizing antibiotic efficacy and cost- effectiveness.  They will work toward identification of adverse drug reactions and plans to avoid future events as well as improvements in patient care through potentially decreased length of stay.  They will participate in antimicrobial Formulary optimization to help realize further cost benefits and improve congruence with evidence based practice.

      Primary Duties and Responsibilities:

    • Practices pharmacy with a focus on antimicrobial management in accordance with the laws, rules and regulation of the Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug Administration, and the Drug Enforcement Administration and in accordance with established policies and procedures.  Maintains certification and credentials in accordance with the Board of Registration and submits proof to department of certification annually.
    • Assumes responsibility for daily activities of the Stewardship program including pre-prescriptive approval reviews, post prescriptive review and feedback, Vigilanz alerts relative to drug duplication, bug-drug mismatches, therapeutic drug monitoring and intravenous to oral conversions.

    Reviews patient specific data and identifies opportunities for intervention such as
    • De-escalation based on microbiologic data
    • Dosing optimization based on microbiologic data/patient specific PK
    •  Medication adjustments to avoid clinically significant Interactions 
    Assists the Antimicrobial Management Directors in the preparation of monthly/quarterly summaries of interventions to include:
    • Quantification and qualification of interventions by: acceptance rate, cost avoidance, service and unit specific trends.
    Participates in the planning and implementation of clinical staff development for pharmacists toward antimicrobial management. Serves as a mentor to the pharmacy staff in terms of pharmacy practice and patient care. 
    Participates actively on the Antibiotic Subcommittee to P&T Committee through the following activities.
    •  Submits items for the monthly agenda.
    •  Assists in the preparation of anti-infective drug Formulary reviews.  This will include literature searches and retrieval for a summative and evaluative review of basic science and clinical literature.  A written summary in a format that is standardized and complete will need to be compiled. Within this summary, a recommendation for the addition or substitution of drugs on Formulary will be written a rationale according to hospital utilization and published evidence.
    • Assists in the performance of drug utilization reviews by any of the following categories: 
      • Drug based: drugs with high use, drugs associated with high costs, drugs with narrow safety margins.
      • Pathogen based: pathogens with high frequency, pathogens associated with high costs.
      • Resistance trending: identify potential relationships between utilization and local resistance.
    • Assists in Guideline development relevant to antimicrobials and/or infectious diseases through work with multidisciplinary groups to devise and implement drug specific guidelines and disease state specific guidelines, particularly around core measure initiatives.
      • This will involve literature searches and retrieval for a summative and evaluative review of basic science and clinical literature.
      • From this, a gradation of evidence based practice will be compiled
      •  A written draft will be prepared.

    Participates in Guideline Quality Assurance and dashboard metrics through identification and tracking of key core measures as well as preparation of monthly/quarterly/annual report summaries.


    Works collaboratively and systematically to review the current standard antimicrobial susceptibility reports based on site of infection and pathogen. Assists in the evaluation of the impact of panel changes on antimicrobial ordering.

    Contributes to physician education programs aimed to build awareness about institution-specific best practice relative to antimicrobial utilization.

    Works with the Associate Director to test and continually evaluate clinical decision support changes in cPOE relative to antimicrobial utilization.

    Assists the Medical and associate Director to ensure the components of Clinical Pharmacy Services relative to antimicrobials meet the requirements necessary for TJC accreditation.  

    Assists in the implementation of departmental short and long term goals, and participates in accomplishing agreed-upon goals.  Assists in the coordination, implementation, administration and quality assessment of clinical pharmacy programs.

    Qualifications/Skills & Knowledge Requirements:

    Required Qualifications:

    1.    Bachelor's degree in Pharmacy required. Doctoral degree in Pharmacy (PharmD) preferred.

    This job will expand our already existing team. With the addition of a second pharmacist to the team, dedicated time will be split between Clinical ID and Antimicrobial Stewardship. Clinical ID time will be spent supporting and rounding with Infectious Diseases consult team(s). UAMS has 3 ID consult teams: general consults, transplant ID, and orthopedic infectious diseases. Additionally, there is an Internal medicine/ID lead team that is attended by ID. While providing ID support, there will be experiential teaching opportunities with the UAMS College of Pharmacy and precepting for the UAMS PGY-1 Pharmacy Residency. There will also be dedicated time with the Antimicrobial Stewardship Program (ASP). The UAMS ASP has been in place for 10 years, and utilizes different strategies and tools to optimize antimicrobial use.

    UAMS is a 500 bed academic medical center/level 1 trauma center located in Little Rock, AR and is the only academic medical center in the state of Arkansas. UAMS has a bone marrow transplant program and solid organ transplant program, providing a diverse patient population. There are 4 PGY-1 pharmacy residents, and a critical care PGY-2 pharmacy resident, with a hope to start a PGY-2 Infectious Diseases pharmacy residency.
