    Regions Hospital is looking for an experienced Infectious Diseases (ID) Clinical Pharmacy Specialist to coordinate our Antimicrobial Stewardship Program (ASP) activities and other ID initiatives for Regions Hospital as well as the entire HealthPartners health system. The Regions Hospital ID Clinical Pharmacy Specialist will join a team of systemwide ID pharmacists and work collaboratively with ID physicians, infection prevention, microbiology, clinical pharmacy, medicine, nursing, and information technology to improve care delivered across our system of eight hospitals.     Overview:

    Minimum Qualifications:

    This 1.0 FTE position currently designed as 0.5 FTE for pediatric Infectious Diseases/Antimicrobial Stewardship and 0.5 FTE within NICU at Lee Health-Golisano Children’s Hospital of Southwest Florida.  We are located a few miles from the beaches of Southwest Florida including Sanibel and Captiva.     The Ambulatory Care Clinical Pharmacist for HIV and Infectious Diseases collaborates to provide medication management, medication access, and patient education in order to optimize clinical outcomes.     Job Description: MGH Department of Pharmacy is seeking a Clinical Coordinator in Infectious Diseases to lead a team of 4 ID Pharmacists and 1 PGY2 Pharmacy Resident. The Clinical Coordinator will co-lead and support the continued advancement of the Anti-infective Stewardship Programs (ASP) at MGH.  This team leader will help advance the mission of the program while serving as a liaison between the MGH Infectious Diseases and various other pharmacy teams (critical care, internal/hospital medicine, oncology, etc.) and providing direct patient care within the multidisciplinary Infectious Diseases service(s).

     

    Clinical Pharmacy Coordinators are team leaders who assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy.  They design, implement, and lead new or existing pharmacy programs, quality improvement changes, and continuous quality assurance initiatives (including stewardship activities and restriction/surveillance programs) to advance the organization’s medication-use systems. Clinical Pharmacy Coordinators proficiently provide direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These clinicians are aligned with target interdisciplinary programs and specialty services to lead the delivery of medication therapy management within specialty patient care service(s) and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. They proficiently manage and facilitate the appropriate use of medications according to professional and regulatory standards.     Qualifications

    REQUIRED:

    • Must be eligible for licensure to practice pharmacy in the Commonwealth of Massachusetts
    • Postgraduate training or work experience consisting of meeting at least 1 of the following criteria:
      • 1 year of postgraduate training PLUS 2 years post-training experience in an acute care or ambulatory setting
      • 2 or more years of postgraduate training
      • 5 years of practice experience in an acute care or ambulatory setting
    • Board Certification by examination, or equivalent certification, in the relevant specialty is required
    • Must meet all Board of Pharmacy requirements to enter into a collaborative drug therapy management agreement, when applicable

    The ideal candidate will be responsible for providing pharmaceutical care to patients in the areas of infectious diseases at the University of Illinois Hospital.  The hospital is a 450-bed academic institution located approximately 2 miles west of downtown Chicago. The hospital serves an urban, indigent patient population.

    Duties:
    The clinical pharmacists are integral members of the multidisciplinary teams at UI Health.  The new hire will work with the current ID clinical pharmacist to provide antimicrobial stewardship services and clinical services for the ID-consult and ID-transplant services primarily dayshift Monday – Friday.  The new hire will also be a member of the anti-infective subcommittee and participate in the infection control committee.  The clinical pharmacist duties may include attending daily rounds, medication profile review, identifying medication related problems, evidence-based medication and dosing recommendations, therapeutic drug monitoring, provision of drug information, and patient education.  The candidate will also be responsible for nurse, physician, and pharmacy staff development; coordinating and conducting quality initiative projects and developing collaborative treatment and research protocols.

    The academic responsibilities of the clinical faculty member (Clinical Assistant/Associate/Full Professor) include the provision of didactic lectures and precepting entry level Doctor of Pharmacy students, PGY1 residents and PGY2 residents. Scholarly activities, including presentations and publications, are an expectation as well. Academic rank commensurate with experience.

    Minimum Qualifications:
    PharmD degree from an ACPE accredited Doctor of Pharmacy Program.
    Completion of Post Graduate Year 1 (PGY1) residency required.
    A Post Graduate Year 2 (PGY2) residency or fellowship in infectious diseases or 2-3 years equivalent experience in the specialty area required.
    Must present evidence of, or eligibility for, Illinois licensure as a professional pharmacist by start date.
    Must possess knowledge and skills such that they qualify for a non-tenured clinical faculty position within the Department of Pharmacy Practice at the UIC College of Pharmacy.


    Preferred Qualifications:
