    At Houston Methodist, the Pharmacy Clinical Specialist is responsible for performing advanced clinical pharmacy duties enabling the delivery of optimal pharmacotherapy to patients and facilitating safe, efficacious, and pharmacoeconomically rational use of medications in the population(s) served. This involves daily reactive and targeted proactive medication regimen reviews for patients within the designated area(s) of responsibility or populations served. The Pharmacy Clinical Specialist supports the delivery of pharmacy managed protocols, formulary processes and medication reconciliation activities while providing education to fellow pharmacists, nurses, medical staff. Additional duties include precepting pharmacy students and pharmacy residents on clinical patient care rotations within areas of responsibility and contributing to fulfilling the organization’s mission to provide innovative, personalized, cost-effective pharmaceutical care in a culture dedicated to quality and safety.

    PEOPLE - 20%

    SERVICE - 20%

    1. Processes all orders/requests accurately and efficiently, verifying appropriateness of all drugs in reference to the patient’s diagnosis, allergies, height and weight. Exercises good judgment in patient care decisions per performance standard and documents patient care activities and patient outcomes impacted by pharmaceutical care and clinical interventions. (EF)
    2. Provides patient-specific and non-patient specific drug information support regularly. (EF)
    3. Serves as a member on one or more Medical Staff Committees and actively contributes to accomplishing the goals of the committee. (EF)

    QUALITY/SAFETY - 40%

    1. Collaborates with other health care professionals to manage patient’s drug therapy for maximal effectiveness while minimizing side effects, adverse drug reactions, toxicities, drug interactions, and incompatibilities using established internal and external guidelines. (EF)
    2. Provides pharmaceutical care at the fundamental to advanced level for patients within the designated practice area(s). Activities include, but are not limited to, proactive drug therapy reviews and monitoring, patient counseling, transitions of care management including medication acquisition process support, and management of pharmacist-based dosing protocols. (EF)
    3. Participates in quality management initiatives such as, but not limited to: medication use evaluations, adverse drug reaction and medication error reporting, clinical interventions, medication center reviews, and inventory management documentation and reporting. (EF)

    FINANCE - 10%

    1. Contributes to the provision of pharmacoeconimically sound medication therapy by upholding HM Formulary standards in area(s) of responsibility and avoiding medication-related complications. (EF)
    2. Self-motivated to independently manage time; effectively prioritizing tasks for self and for others within the specialty area of focus. (EF)

    GROWTH/INNOVATION - 10%

    1. Presents in-services for pharmacy, medical, nursing, and other staff members in area(s) of responsibility. Develops educational materials and provides educational programs for the pharmacy staff and other hospital personnel. (EF)
    2. Seeks opportunities to expand one’s learning beyond fundamental competencies with a focus on continual development as a Clinical Specialist. (EF)
    3. Contributes to the design, analysis and reporting of quality improvement program findings.
    4. Participates in research projects to advance organizational quality of care and the pharmacy profession. Submits research and scholarly work for publication, platform presentations, and/or poster presentations.

    Qualifications:

    EDUCATION REQUIREMENTS

    • Graduate of education program approved by the credentialing body for the required credential(s) indicated below in the Certifications, Licenses and Registrations section
    EXPERIENCE REQUIREMENTS
    • Must have one of the following:
    • Successful completion of an ASHP-accredited post-graduate year one (PGY1) residency training program or equivalent experience (i.e., three years of pharmacy experience per residency year) required; ASHP-accredited post-graduate year two (PGY2) or fellowship preferred
    • Three years of clinical practice experience
    • Meets HM standards of an ASHP-Qualified Residency Preceptor within one year of employment

    CERTIFICATIONS, LICENSES AND REGISTRATIONS REQUIRED

    • Texas Pharmacist License or eligible for Texas Pharmacist License; must obtain licensure within 90 days of employment
    • American Heart Association Basic Life Support (BLS) certification within 90 days of employment
    • American Heart Association Advanced Cardiovascular Life Support (ACLS) certification within 90 days of employment

    WHAT YOU WILL DO

    • Collaborate with allied health disciplines to provide quality patient care.

    Clinical Pharmacist Specialist, Infectious Diseases and Antimicrobial Stewardship

    Position Summary:

    The Department of Pharmacy is seeking a pharmacist specialist whose primary role will be providing infectious disease (ID) focused pharmacotherapy services to hospitalized patients. The pharmacist will be responsible for antimicrobial stewardship (AMS) activities at Newport Hospital (NH) while also providing AMS and ID Consult coverage at The Miriam Hospital (TMH).

     Position activities include:

    • Provide direct pharmaceutical care services by promoting the safe and effective use of anti-infective agents
    • Coordinate and maintain the AMS program in collaboration with the AMS physician directors and affiliate-wide AMS/ID pharmacists.  These responsibilities include, but are not limited to:
      • Conduct daily AMS activities including: prospective audit of antimicrobial use, review of all positive blood culture results, respiratory viral panel results, and Accelerate Pheno® results for Gram negative rod bacteremia with real-time intervention and feedback (12 months NH, 6 months TMH)
      • Implement AMS initiatives to promote optimal antimicrobial use and measure impact of interventions
      • Report on applicable AMS metrics, including antimicrobial utilization and identify opportunities to optimize antimicrobial use and patient care
      • Participate in anti-infective/AMS quality improvement projects in collaboration with pharmacy, infectious diseases, clinical microbiology, infection prevention, nursing, and quality
      •  Assist in developing and maintaining the anti-infective formulary, anti-infective guidelines and use evaluations, infectious disease related clinical pathways, and order sets
      • Serve as an active member of the Lifespan Antimicrobial Stewardship Committee and Infection Prevention Committee
      • Ensure TJC, CMS and other AMS regulatory compliance
    • Round with infectious diseases consult service(s) (~16 weeks at TMH)
    • Attend Wednesday and Friday ID Conference and Micro Rounds, act as a resource to the inpatient ID providers and fellows regarding drug therapy and monitoring
    • Education-related activities include:
      • Serve as a preceptor to pharmacy residents (PGY1 and PGY-2), APPE pharmacy students, and infectious diseases physician fellows
      • Provide didactic lectures to local pharmacy, nursing, and physician assistant schools
      • Provide in-services to pharmacists, providers, nurses, and other healthcare staff on but not limited to AMS, protocols, guidelines, and department initiatives
    • Serve as a liaison between the Department of Pharmacy and Division of Infectious Diseases
    • Maintain competency in the pharmacy dispensing system through participation in practice coverage in the inpatient pharmacy.  Weekend rotation is one weekend a month and two holidays
    • Demonstrate ongoing professional development including meeting continuing education requirements, keeping abreast of current literature as it relates to clinical, financial and safety outcomes in the area of infectious diseases and antimicrobial stewardship
    • Contribute to pharmacy practice and medical community through research project presentations at state/national meetings and/or publication in reviewed journals.  Specialist may serve as a mentor on resident research projects

    Qualifications:

    • Graduate of an accredited College of Pharmacy, Doctor of Pharmacy degree
    • Licensed to practice pharmacy in the State of Rhode Island or eligible for licensure
    • Completed an ASHP-Accredited PGY1 pharmacy residency and an ASHP-Accredited PGY2 in infectious diseases or applicable fellowship or equivalent experience
    • Ability to function independently and collaboratively within a team setting. Highly motivated; effective communication, problem solving, and organizational skills required

    Reporting Structure:

    The infectious diseases clinical pharmacist specialist reports to the Director of Inpatient Pharmacy at Newport Hospital and collaborates closely with the Antimicrobial Stewardship Directors.  Other key collaborators include infectious disease pharmacists, other clinical pharmacists, infectious disease physicians, microbiology and infection control departments. This individual will also interact with other various departments including but not limited to information technology, administration, physician groups, and nurses.

    ID Clinical Pharmacy Team at Lifespan: 

    We currently have six Clinical Pharmacist Specialists in Infectious Diseases across the Lifespan System: three Antimicrobial Stewardship/Inpatient ID pharmacists (one specialist at The Miriam Hospital and two at Rhode Island Hospital) and three ID/HIV pharmacists at The Miriam Hospital ID & Immunology Clinic. The ID pharmacists work closely as a team and collaborate with one another to ensure optimal transitions of care between the inpatient and ambulatory settings. Additionally, the team works together on projects, guidelines, protocols, and implementation of system-wide initiatives. The ID pharmacists are members of the Lifespan Antimicrobial Stewardship Committee and attend Infection Control meetings regularly at their respective sites.

    Lifespan Health System and Antimicrobial Stewardship Program (ASP):

    A not-for profit organization, Lifespan is Rhode Island’s first healthcare system committed to providing patient-centered care.  Lifespan health system has five hospitals in Rhode Island including Rhode Island Hospital, The Miriam Hospital, Newport Hospital, Hasbro Children’s Hospital, and Bradley Hospital.  Lifespan Hospitals utilize Epic for their electronic medical record system.

    Newport Hospital is a not-for-profit community hospital with 129 licensed beds offering acute care and community health services to people in Newport County, Rhode Island. Newport Hospital provides a range of services including but not limited to emergency care, general medicine, surgical services, intensive care, inpatient/outpatient rehabilitation, inpatient behavioral health, and birthing services.

    The Miriam Hospital is a not-for-profit community teaching hospital with 247 licensed beds affiliated with The Warren Alpert Medical School of Brown University.  The Miriam Hospital provides a range of services including but not limited to general medicine, emergency medicine, surgical services, and intensive care. In addition, The Miriam Hospital is a Primary Stroke Center, a Center of Bariatric Surgery, and Total Joint Center.

    The Brown Infectious Diseases division has 34 active providers within the Lifespan system. Rhode Island Hospital includes four inpatient infectious diseases adult consult services: three general ID services (teaching and non-teaching) and one ID transplant and hematology/oncology service. The Miriam Hospital has three infectious diseases adult consult services: two general ID services (teaching and non-teaching) and an immunology/HIV service. Newport Hospital has one ID consult non-teaching service.

    The Antimicrobial Stewardship Program (ASP) at Lifespan was implemented in 2014 and encompasses a multi-disciplinary approach. The Lifespan Antimicrobial Stewardship Committee includes representation from the Division of Infectious Disease, Pharmacy, Nursing, Infection Prevention, and Microbiology from ASPs across Lifespan hospital affiliates.  Prospective audit and feedback (including blood culture review) and restricted antibiotics are core strategies for the ASP.  AMS rounds are conducted daily with the ASP directors. In addition, the ASP has a collaborative relationship with microbiology and is instrumental in implementing rapid diagnostic initiatives.

    • Assists with transitions of care including medication reconciliation, medication education, prior authorization, access to medications, sustainable coverage, and/or scheduling follow-up appointments

    We are seeking applicants for a Clinical Pharmacy Specialist position in Transplant Infectious Diseases at William P. Clements Jr. University Hospital at UT Southwestern Medical Center.

    This position is part of the pharmacy department and functions as the pharmacy expert on the interdisciplinary transplant ID consult team. The transplant ID specialist serves as a leader for the department in their specialty area of practice through organizing and executing appropriate departmental initiatives, maintaining transplant specific aspects of the antimicrobial stewardship program, and providing education and teaching for pharmacy colleagues, providers, and trainees at all levels of practice. Day-to-day activities include rounding on the inpatient transplant ID service; evaluating, monitoring, and modifying drug therapy to optimize patient outcomes and safety; and precepting pharmacy residents and/or students. Additionally, there is dedicated time to serve on the inpatient antimicrobial stewardship team and opportunities to lead or participate in research and scholarship. UTSW has three ID consult services (teaching and non-teaching general ID consult teams and transplant ID consult team), an OPAT program, and an antimicrobial stewardship program. Among these teams, there are currently four ID clinical pharmacy specialists.

    This is a full-time, benefit-eligible position that will be scheduled primarily day shifts with occasional need to take calls at other times, as needed, at the William P.     EXPERIENCE and EDUCATION

    Must meet one of the following requirements:

    • Completion of both PGY1 and PGY2 from an ASHP-accredited residency program; OR
    • Completion of PGY1 from an ASHP-accredited residency program and 4 years as a pharmacist in a progressive clinical practice within  infectious diseases; OR
    • Completion of a specialty fellowship infectious diseases; OR
    • 6 years as a pharmacist in a progressive clinical practice within infectious diseases.

    Bachelor of Science (BS) degree or Doctor of Pharmacy (PharmD) degree from an ACPE accredited school of pharmacy is required.

    Eligible for licensure by Texas State Board of Pharmacy (TSBP) is required and Texas license with TSBP within 30 days of date of hire. Current Basic Life Support (BLS) certification with course accredited by the American Heart Association (AHA). BPS board certification is preferred.

    Working knowledge of computers and data-entry skills are essential. Ability to communicate clearly in English (speaking, reading, and writing) is required. Good general health and a strong ability to cope with stress in a faced-paced environment is a must. Position requires moderate physical effort: prolonged standing/walking, occasional lifting/carrying (up to 25 lbs.), reaching, bending, crouching, etc. (OSHA Category III).

     

    JOB DUTIES 

    1. Performs job responsibilities in accordance with organizational and departmental policies and procedures, performance improvement objectives and environmental/infection control standards.

Required clinical activities include a) Yearly development of a cost-saving initiative, quality, and/or operations improvement with manager sponsor, MUE or drug class review with manager sponsor b) yearly ACPE/non-ACPE presentation (rotating between ACPE/non-ACPE) c) individual CE in areas of specialty (6 hours) d) P&T or Subcommittee Attendance (50% Attendance requirement) d) Annual submission of research project on Research Submission Template (Due April 1st every year).
    6. Actively promotes pharmacy. Coordinates and participates in multidisciplinary in-services/meetings, publishes articles (i.e. staff newsletter), individualized drug information, rounds, consults, etc. Maintains high visibility/availability through participation in in-services, meetings and daily activities. Participates in the education/experiential training of pharmacy students and pharmacy residents.
    7. Actively manage P&T initiatives through monitoring of documented interventions. Verifies data, develops measures and reports outcomes (Performance Improvement). Supports cost savings initiatives. Has active role in assessment and selection of cost-effective pharmaceuticals. Reviews formulary addition request and develops monographs for presentation. Manage the formulary system and procedures for acquisition of non-formulary product.
    8. Consistently works and problem solves independently and effectively. Takes charge when necessary and is looked to for direction. Makes the right decisions and uses good judgment. Cross trained and multifunctional maintaining competency in multiple areas. Mentors clinical staff pharmacists and supportive personnel.
