Details: The Johns Hopkins Home Care Group is seeking a motivated individual to assume a clinical leadership position in infectious diseases ambulatory and home care practice. The individual will work with an interdisciplinary team to provide clinical management and oversight for outpatient parenteral antimicrobial therapy (OPAT) services, including supporting transitions of care services from acute care to home infusion and skilled nursing facilities under a collaborative practice drug therapy management agreement. The individual will develop and apply medication use policies, lead quality improvement initiatives for OPAT services, support the development of ambulatory antimicrobial stewardship services, and participate in research activities. The individual will work closely with physicians, nurses, and pharmacists across the continuum of care to ensure safe, timely, and effective OPAT treatment and monitoring. She/he will serve as a preceptor for student pharmacists and pharmacy residents of ASHP accredited PGY-1 and PGY-2 residency programs. Faculty appointments are available through affiliations with the University of Maryland College of Pharmacy and the University of Notre Dame of Maryland College of Pharmacy. A PharmD degree is required, PGY1 Pharmacy Practice residency training (or equivalent experience) is required, and PGY2 Infectious Diseases residency training with OPAT and home infusion experience is preferred.
