    POSITION SUMMARY

    The Internal Medicine/Antimicrobial Stewardship Clinical Pharmacist Specialist provides comprehensive pharmaceutical care to all patients through integrated drug distribution, clinical services, and education.

    POSITION REQUIREMENTS

    Formal Education / Training:

    Earned a Pharmacy diploma or degree from an ACPE accredited college or university. Completed an ASHP accredited PGY-2 Internal Medicine or Infectious disease residency or PGY-1 Pharmacy Practice residency with three years relevant clinical experience or five years relevant clinical experience. Must have a current license from the State of Tennessee. Board certification in Infectious Disease within one year of hire. ACLS within six months of hire.

    Workplace Experience:

    Hospital or related experience required.

    Equipment and Skills Training:

    Meditech Pharmacy Module computer system, Automated Dispensing Cabinet Workstation, calculators, personal computers and printers, laminar flow hood, PDAs.

    Physical Environment:

    Work in an air-conditioned room and/or clean room or office. View and accurately type on a computer terminal for at least a two hour period. Ability to handle frequent interruptions and adapt to changes in workload and work schedule.

    Physical Effort:

    Occupational Hazards:

    PERFORMANCE STANDARDS

    1. Provide operational support on the floors/wards and Central Pharmacy as determined by departmental need; Review medication orders for accuracy and appropriateness; Order entry for assigned areas; Resolves drug distribution related issues; Resolves automated dispensing cabinet related issues; Covers assigned shifts as determined by departmental need, including evenings, weekends and holidays

    2. Discuss medication order clarifications with the prescriber, document any changes in patient and pharmacy records, and inform others of medication order changes

    3. Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions

    4. Prevent, detect, monitor, document, and report adverse drug reactions and medications errors. Participate in the drug therapy management in emergencies situations. Identify and conduct patient safety and process improvement initiative as need arises.

    5. Promote the use of the formulary by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary drugs when necessary. Conduct drug formulary reviews and present to appropriate committees.

    6. Promote and demonstrate rational, cost-effective drug therapy. Identify and implement drug therapy where positive pharmacoeconomic impact could be achieved. The ultimate goal is to develop and assure cost effective clinical pathways.

    7. Leads Antimicrobial Stewardship Program Team and serves as pharmacy chair. Responsible for regulatory reporting of antibiotic use through NHSN; responsible for prospective review of patients on antibiotics and blood culture results; facilitates the activities of the pharmacists with regards to antibiotic stewardship. Works with hospital epidemiologist/infectious disease specialist on appropriate use of hospital antibiogram and antimicrobial agents.

    8. Develop and conduct target drug programs and drug usage evaluations as needed and report results to the Pharmacy and Therapeutics Committee. Participate as an active member of Pharmacy and Therapeutics Committee.

    9. Document clinical interventions and cost avoidance as part of the departmental documentation program. Monitor the use of expensive medications to assure there is use consistent with approved criteria.

    10. Anticipate changes in drug therapy and report the potential impact it will represent to the overall hospital and drug budget.

    11. Take a leadership role in the teaching programs of the Department of Pharmacy Services and Colleges of Pharmacy as they are relate to Internal Medicine and Antimicrobial Stewardship, precept students and residents. Contribute and assist in editing the monthly pharmacy newsletter.

    12. Provide staff development support to nurses and physicians.

    Clinical Pharmacy Manager – Infectious Diseases / Antimicrobial Stewardship

One position is available at the NewYork-Presbyterian/Weill Cornell Medical Center campus, which will join an already successful team of seven additional clinical pharmacy managers in infectious diseases across the healthcare system. Candidates will be expected to design, implement, and monitor pharmacotherapy of general and immunocompromised adult ID patients through multi-disciplinary rounds or daily antimicrobial stewardship activities, and provide education to physicians, nurses, pharmacists, and other members of the health care teams.     Scope of Position

    In cooperation with physicians, advanced practice providers, nursing and other health care providers, acts as a resource and provides answers to drug information questions pertaining to OPAT and complex oral antimicrobial therapy. Participates in the ambulatory management of patients receiving OPAT and/or complex oral antimicrobial therapy for patients of the Infectious Diseases Care in McCampbell Outpatient Care clinic. Participates in appropriate hospital and/or departmental committees relevant to practice area, specifically antimicrobials.

    Position Summary

    This position functions to coordinate all aspects of patients' pharmaceutical needs related to OPAT and complex oral antimicrobial therapy. Duties include but are not limited to: proactively participating in patient clinic visits in the OPAT/infectious diseases outpatient clinic; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; ordering and monitoring appropriate labs;  pharmacokinetic monitoring; participates in transition of care rounds; provides antimicrobial therapy pharmacotherapeutic consults; assists in ensuring patient access to medications; and interactions with specialty pharmacies and home infusion pharmacies as applicable. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.  Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age.  This position is also responsible for supervising pharmacy students, pharmacy residents, and providing educational programs for patients and hospital staff (providers, nurses, medical students, graduate medical education, and pharmacy staff) as applicable. PGY1 and PGY2 Infectious Diseases residency training are preferred.

    UPMC Altoona is a 374-bed acute care teaching hospital in Altoona, Pennsylvania. It is a major hospital within the UPMC Health System. 

    The ID Pharmacist will be responsible for daily stewardship activities and local oversight of the stewardship program. The pharmacist will collaborate with and receive support from UPMC Health System’s Antimicrobial Stewardship leadership to leverage system resources and facilitate stewardship best practices locally. Other activities include: 

    · Utilize ILUM Insight CDSS as part of daily workflow 

    ·  Work collaboratively with local hospital and pharmacy leadership, Infection Prevention, Patient Safety, Clinical Microbiology, and other relevant local leaders to improve antimicrobial stewardship practice and patient outcomes 

    · Participate in both local and health system antimicrobial subcommittees

    Physical Requirements

May be required to frequently stretch, bend, squat, or kneel to perform job. Utilizes appropriate personal protective equipment (PPE) to minimize exposure to hazardous agents.     POSITION SUMMARY
    This position actively participates on the . healthcare team as a pharmacotherapy expert to provide quality patient care in area of specialty. Participates in the optimal selection of drug therapy and designs a regimen to maximize therapeutic benefit and minimize toxicity. The Acute Care Clinical Pharmacy Specialist is accountable for on-going evaluation and documentation of all components of pharmaceutical care delivered to patients. Conducts educational activities, applies principles of performance improvement, and participates in Pharmacy and Therapeutics Committee/Clinical Consensus Group activities as appropriate. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients being served. Accountable for departmental, and individual patient outcomes by assessing and evaluating data reflective of the patient’s status, interpreting the appropriate information needed to identify each patient’s requirements, and providing pharmaceutical care based on individual needs. Responsible for the delivery of pharmaceutical care to patients using the medication use process. Oversees the activities of the pharmacy technicians, coordinating workflow, and the checking of medications prior to being dispensed. Adheres to local, state, and federal laws regarding the profession of pharmacy practice.

    CORE FUNCTIONS
    1. Critically evaluates the management of disease states and adjusts or recommends adjustments to therapy to   improve patient medication regimens. Ensures appropriate and cost-effective medication therapy. Areas of focus may include but are not limited to evaluating disease state management, medication management, and adverse drug reactions. Adheres to all clinical practices approved by Pharmacy and Therapeutics Committee/Clinical Consensus Group (e.g., collaborative practice agreements). Accurately and completely documents patient assessment data and clinical interventions.

    2. Drug information expert in area of specialty and may act as a system resource. Provides detailed drug information and education to healthcare team, pharmacy, patients, and community. Provides education for pharmacy staff, students, and residents where applicable.

    3. Reviews and evaluates medication orders for appropriateness and completeness with respect to patient data including age, height, weight, diagnosis, allergy, laboratory values and other pertinent patient information. Complies with directives of the Pharmacy and Therapeutics Committee/Clinical Consensus Group (e.g. therapeutic substitutions, formulary management).

    4. Applies the principles of continuous quality improvement consistent with job expectations. Incorporates quality improvement principles into other activities and facility/system-based projects (i.e. data collection, documentation, med error and adverse drug event trending). Participates in drug use evaluations and facility level medication safety oversight.

    5. Participates in local and system-based committees (e.g. area of specialty or other related area) and effectively communicates to stakeholders. Participates in the development of innovative drug-related patient care practices in their specialty at both institutional and system level. Serves as a resource and mentor to other pharmacists, students, residents, and other health-care providers at both the facility and system level.
     

    6. Meets ASHP preceptor qualifications. Participates as primary preceptor to residents where applicable. When needed, serves as the Program Director for the institution’s Pharmacy Practice Residency Program. Serves as an educator, resource expert, and mentor to other pharmacists, students, residents, and/or other healthcare providers.

    7. Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Provides all customers of Banner Health with an excellent service experience by consistently demonstrating our core and leader behaviors each and every day.

    MINIMUM QUALIFICATIONS
     

    A Doctorate in Pharmacy degree and at minimum completion of a PGY1 residency program. Without residency training a minimum of 3 years of health-system progressive clinical pharmacy experience (evaluating disease state & medication management e.g. thromboembolic, diabetes, oncology, pharmacokinetics, pain management & nutrition, investigational studies, etc).

    Additional minimum qualifications board certification (e.g. BCPS, BCOP) or equivalent national association/organization approved certification (e.g. ASHP, SOCRA etc.). Candidates who are not Board Certified will be given 18 months to achieve certification.

    Requires pharmacist licensure in the state of practice and a degree in Pharmacy from an accredited College of Pharmacy. If not licensed in the state of practice, must have completed the Board of Pharmacy application process within 30 days of job offer. Must be licensed within 90 days of hire unless the delay is due to the State Board of Pharmacy. May be required to possess a valid driver’s license and be eligible for coverage under the company’s auto insurance policy.

    A current and unrestricted Pharmacist license (not intern) is required at the time of hire for BSS applicants. May be required to possess a valid driver’s license and be eligible for coverage under the company’s auto insurance policy.

    Must possess good organizational, problem solving and computer skills. Must possess good written and verbal communication skills for interfacing with all levels of staff, Physicians, and other contacts. Must be able to use reference tools to evaluate patient care and answer questions posed by customers. Contributions to the profession and organization are expected.

    The Infectious Diseases Clinical Pharmacist will:

    • Be responsible for monitoring the appropriate use of inpatient restricted antimicrobials
    • Provide clinical support to providers on the use of antimicrobials and serve as a liaison between the Department of Pharmacy Services and the Division of Infectious Diseases
    • Develop, maintain, and monitor clinical pathways, protocols, and guidelines
    • Oversee antimicrobial non-formulary requests
    • Participate in health-system and institutional Antimicrobial Stewardship committees
    • Serve as a preceptor for the PGY1 Pharmacy program
    • Identify and execute cost-savings initiatives

    Completion of an ASHP-accredited of a PGY-2 specialty residency training in Infectious Diseases or equivalent experience is preferred.

    Clinical Pharmacy Specialist – Antimicrobial Stewardship and Infectious Disease

    The Antimicrobial Stewardship Clinical Specialist pharmacist will oversee the Antimicrobial Stewardship Program across the facilities located in North Dakota and Minnesota who are part of the Midwest Division of CommonSpirit Health. The position located in Bismarck, North Dakota will be primarily stationed at St Alexius Health Bismarck Medical Center but will have remote monitoring responsibilities for other facilities.  Relationship building with remote facility hospital leadership, providers, and caregivers will be essential with occasional travel.  It is anticipated that travel would be ~10% of time or less.

    Responsibilities

    • The clinical specialist will provide pharmaceutical care services by promoting the safe and effective use of antimicrobial agents. 
    • Responsible for monitoring the appropriate use of targeted antimicrobials in inpatients.
    • Works independently and in collaboration with the Division of Infectious Diseases, when needed, to design and implement patient-specific drug regimens that optimize appropriate antibiotic coverage in prophylaxis, empiric use, and documented infections. 
    • Responsible for recommending improved antimicrobial regimens in patients who require them through the use of written and oral communication. 
    • Provides drug information services to the physicians of the hospitals on the use of antimicrobials, serves as a liaison between the Department of Pharmacy Services and the Division of Infectious Diseases. 
    • Serves as clinical reference through the provision of educational in-services, drug utilization reviews, guideline development, and informal consults. 
    • Enforces institutional policies and procedures related to medication ordering, dispensing, storage and administration.
    • Develops protocols together with Antimicrobial Stewardship subcommittee that promote the prudent use of antimicrobial agents, and will assist in formulary development by providing clinical expertise in the selection of antimicrobials.
    • Assists with leading the division Antimicrobial Stewardship Program (ASP) and the division Antimicrobial Stewardship subcommittee, together with the Antimicrobial Stewardship physician, and the Division Director of Clinical Pharmacy Services. 
    • Responsible, together with Antimicrobial Stewardship physician and ASP, for improving and maintaining outcomes of the ASP at all CSH facilities across North Dakota and Minnesota.  Desired outcomes include but are not limited to antimicrobial cost savings, reducing inappropriate antimicrobial days of therapy for hospitalized patients, reducing hospital acquired infections (including C. difficile), and other outcomes/goals of the ASP program. 

    QUALIFICATIONS -

    Education:

    Required: Pharm.D degree.

    Licensure/Certification:

    Required: North Dakota Pharmacist License with willingness to get licensed in other states (Minnesota)

    Preferred: Antimicrobial stewardship or infectious disease experience, preferably with PGY2 in Infectious Disease or Infectious Disease Board Certification or equivalent experience.

    Infectious Diseases Pharmacist Practice Lead

    Overview

    Atrium Health Wake Forest Baptist (AHWFB) is currently searching for motivated candidates for an Infectious Diseases Clinical Pharmacist.  This position is an integral part of a large pharmacy team and progressive group of pharmacy leaders who provide comprehensive pharmacy services throughout the medical center. The Department of Pharmacy’s goal is to enhance its pharmacy operations, advance clinical pharmacy programs, and expand interdisciplinary patient centered care services and pharmacy residency programs. 

    Since 1982, the Infectious Diseases Clinic at Wake Forest has provided HIV medical care for a burgeoning patient population with multiple co-morbidities and significant psychosocial complexity. As one of the largest HIV providers in the state, comprehensive HIV care is provided in the clinic, including primary medical care, full laboratory services, psychology, nutrition, and supportive services. A number of infectious diseases requiring antimicrobial therapies are managed by clinical pharmacist practitioners working in the clinic, including HIV, hepatitis B and C, mycobacterial infections, and infections requiring long-term intravenous and oral antibiotics.

    Responsibilities

    The Infectious Diseases Clinical Pharmacist will:

    • Proficiently provide direct patient-centered care in all aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care.

    • Provide care on a rotating basis among three service areas: Infectious Disease Clinic/Outpatient Antimicrobial Therapy, Inpatient Infectious Disease Consults, and Antimicrobial Stewardship. The pharmacist will be an integral member of a team of clinicians who operationalize prior authorization of restricted antimicrobials for the health system.

    • Collaborate with infectious diseases physicians, pharmacy clinical directors/managers, infection prevention, clinical microbiology staff and other professionals.

    • Develop interventional strategies to maximize appropriate anti-infective utilization and improvement in patient outcomes, including but not limited to guidelines, clinical pathways, restriction criteria, policy development, new laboratory methods and formulary management

    • Establish a means to report and monitor ongoing anti-infective utilization and identified metrics showing effectiveness of the health-system stewardship program

    • Identify opportunities for improvement regarding anti-infective use, including but not limited to performing medication use evaluations, conducting clinical outcomes research, and leading quality improvement projects.

    • Provide ongoing education to pharmacists, physicians, and other clinicians throughout AHWFB by developing educational materials and competencies

    • Serve as a preceptor for the PGY1 Pharmacy, the PGY2 Infectious Disease, and other residency programs

    • Provide experiential mentorship for Doctor of Pharmacy Students

    • Participate in hospital committees, interdisciplinary collaboration, integrated operational support, and departmental leadership

    • External presentations, publications, and professional speaking is strongly encouraged and supported   

    Infectious Diseases pharmacists at WFBH facilitate pharmacy services to patient with infectious diseases and collaboratively support direct patient-centered care.

    Position requirements include:

    1. Education: Doctor of Pharmacy degree from an ACPE accredited College of Pharmacy
    2. Clinical training and experience: PGY1 Pharmacy residency and PGY2 Infectious Disease residency or fellowship are preferred. At least 2 years of clinical experience in infectious disease pharmacy practice, including HIV care, may be considered in lieu of formal training, and exceptional aptitude or knowledge may be considered in lieu of experience.
    3. Licensure/certifications: Licensed or eligibility for licensure to practice pharmacy in the state of North Carolina is required. Board of Pharmacy Specialties (BPS) certification is preferred (BCPS or BCIDP). Eligibility for Clinical Pharmacist Practitioner (CPP) approval is preferred.
