This position will focus on supporting the antimicrobial stewardship program at Winchester Medical Center. The clinical staff pharmacist will provide prospective audit and feedback of patients on select antimicrobials in a patient centered area as well as review of therapeutic alerts including drug-organism mismatches and positive culture results. A staffing component is also required including the following activities: order entry and order verification, pharmacokinetic dosing, renal dosing, and IV to PO conversions. Additional responsibilities in conjunction with the ASP ID Clinical Pharmacist will include precepting pharmacy residents and students, participation on committees, projects and/or research related to antimicrobial use and medication/patient safety issues, and education of the health care team, including pharmacists, nurses, and providers.

Responsibilities:
- Prospective audits of patients on select broad-spectrum antimicrobial therapy in designated patient centered area
- Review therapeutic alerts related to antimicrobial therapy include positive cultures, susceptibility results, and drug-organism mismatches using Theradoc®
- Documentation of antimicrobial interventions in EPIC with use of IVENTS
- Follow-up patients on daily antimicrobial stewardship list
- Assist with pharmacokinetic consults, renal dosing, and IV to PO conversions
- Assist with order entry and order clarification
- Collect outcomes data related to ASP interventions and patient outcomes
- Participate in committees related to antimicrobial therapy: Formulary, P&T Committee, Antimicrobial Subcommittee, and others as needed
- Special Projects may include:
- Participation in development of new antimicrobial stewardship initiatives
- Formulary drug monographs for antimicrobials
- Medication Use Evaluations to review effectiveness and safety of antimicrobial therapy
- Participate in training of students and residents on ASP rotation
- Educate pharmacy staff on antimicrobial therapy, including pharmacokinetics
- Participate in medication and patient safety related activities:
- ADR reporting
- Documenting and reviewing risk reports
- Suggestions for work flow improvement related to antimicrobial therapies

Requirements:
- BS in Pharmacy or Pharm D required.
- Pharmacy Residency or two years of relevant institutional experience required
- Virginia Pharmacy License required (or acquired within 90 days of hire)
- Experience with EPIC electronic health record and Pyxis preferred
