Experience: Applicants must possess a PharmD from a college accredited by the American Council on Pharmaceutical Education and must hold or be eligible for pharmacist licensure in Texas. 

Applicants must meet one of the following requirements:

Completion of both PGY1 and PGY2 from an ASHP-accredited residency program; OR

Completion of PGY1 from an ASHP-accredited residency program and an infectious diseases fellowship; OR

Completion of PGY1 from an ASHP-accredited residency program and 5 years as a pharmacist in a progressive clinical practice in infectious diseases; OR

8 years as a pharmacist in a progressive clinical practice in infectious diseases.
